Wish Referral

Who can refer a child for a wish?
Out of respect for the privacy of the children and families we serve, children who may be eligible to receive a wish can be referred by one of three sources:

Medical professionals treating the child – such as doctors, nurses, social workers or child-life specialists

A parent or legal guardian of the potential wish child

The potential wish child

Who is eligible for a wish?
Any child between the ages of 2 ½ and 18 with a life-threatening medical condition is potentially eligible to receive a wish.

How can I refer a child?
If you would like to refer a child for a wish who lives in Minnesota, you may contact us at info@wishmn.org or by phone at 612-767-WISH (9474).  We will send you a wish referral form to fill out and send back so we may find out if your child is eligible to receive a wish. 

If you would like to learn more about referring a child residing outside of Minnesota please contact Make-A-Wish Foundation® of America at www.wish.org

If you would like to learn more about referring a child residing outside the U.S., please contact Make-A-Wish Foundation® International at www.worldwish.org

If you are not eligible to refer a child, please ask the child's family to visit our Web site or contact their local chapter or international affiliate.

The Make-A-Wish Foundation respects the privacy of the children and families it serves.  All medical information is considered confidential and is not discussed with outside parties unless it is required for the wish and the child's parent(s) or guardian(s) have given their consent.
